1. What is the difference between breast augmentation and breast lifting?

How can I tell which one I need? Breast sagging occurs over time due to many reasons such as age, breastfeeding, pregnancy, and genetic factors. When you come to the examination, first of all, many parameters such as the distance of the nipples to the collarbones, chest circumference, shoulder width, height and weight are evaluated together to determine which surgery you need.

2. How is our operation process?

According to the measurements and drawings made before the surgery, we would have 4 main topics to be defined:

A. Incision place: where to place the breast implant.

The implant can be placed with a 1.5 cm incision under the breast, with an incision from the nipple or with an incision under the armpit. In order to decide on the incision place we take into account the age of the patient, breastfeeding experience, tendency to keloid (bad scar) and social life of the patient.

B. Implant place: where to place the implant.

The implant can be placed in roughly 2 positions: the sub-muscular position or the super-muscular position. The super-muscular position is now classified as dual-plane 1-2-3. In cases where the patient's chest tissue thickness is medium or good, a sub-muscular placing may be preferred. However, in cases where chest tissue is inadequate, a dual-plane is preferred. In our routine practice, we mostly prefer a dual-plane because it looks more natural and hides the implant lines better.

C. Implant shape.

There are 2 basic implant shapes: anatomical (teardrop) and round. Based on the patient's chest structure, weight, shoulder width, and desired chest shape we will have the guidance in the selection of implants.

I always ask my patients the following questions during this selection: If the décolleté is the key point for you, if you no longer want to use padded bras and want breasts with top fullness even without using a bra, it’s better to choose a round implant;

If naturalness is at the forefront for you, choose a teardrop implant if you imagine breasts that are lightly ptotic and don’t look like being operated when naked.

D. Implant size.

In the preoperative drawing, many parameters including the dimensions of the rib cage, shoulder width, waist width, height and weight can be calculated and the patient can be told an implant size range. For example, minimum 250cc and maximum 350cc implant can be placed. Based on the previous examples (to be shown) of patient’s results who already had the surgery, we will identify which size is closer to the patient's will together.

3.Is there a lot of pain after surgery, when can we return to our daily life?

You spend one night stay in the hospital after the operation. At the end of the surgery, you will be put on your new bra and you should not take off your bra for 1 week. Therefore, you can not take a shower for a week. After the surgery you will have drains which are removed 24 hours. After 24 hours post-operatively, the pain is almost gone, but a week of serious rest is required. Because, even during light exercises such as standing up and walking within this 1 week, there may be a feeling of pain and tension in the chest. This pain may be slightly more in surgeries where sub-muscular plan is preferred. After a week, all kinds of daily activities except heavy lifting can be restarted.

4. What can be done to reduce pain after surgery?

In some eligible patients, intercostal block can be performed under ultrasound after surgery. Thanks to this block, patients can go through this one week much more comfortably. If it is rested for 1 week after the operation, the possibility of feeling pain will not be much.

5. Will this surgery prevent me from breastfeeding in the future? Will I have numbness in my nipples?

Breast augmentation surgery will not prevent you from breastfeeding, this surgery has nothing to do with the milk ducts or glands from a distance. If the place where the implant will be placed is selected as the nipple, it is unlikely that you may experience a 10-20% loss of sensation in your nipple. Therefore, in our routine practice, we prefer the incision of the lower breast fold in every patient under the age of 40.

6. When can I start sports, swim and sunbathe after the surgery?

Light sports can be started, including walking, yoga and some pilates movements, after the first week after the surgery. After the first month, all kinds of activities such as light-heavy sports, swimming, diving can be done. Again, after the 1st week, you can sunbathe with a protective bra.

7. Do I need to change my breast implant in the future? What is capsule formation?

Most of the new generation implants are lifetime, they may need to be replaced for two reasons, first when you want to enlarge or shrink your implant, and second when there is capsule formation around the implant. Capsule formation is the formation of a sheath that surrounds the implant and distorts the shape of the implant, which can occur in 1-2 patients per hundred thousand after an average of 5-10 years after surgery. Numerous scientific studies have been conducted on the causes of capsule formation, some of which are the choice of implants with a rough surface, blood accumulation in the postoperative area (hematoma), and infection. In such a case, the implant should be removed, the capsule cleaned and a new implant placed. Capsule formation is a very rare situation and if all precautions are taken to prevent capsule formation, this possibility is almost zero.

8. Is it true that these implants cause breast cancer?

Not true. Although lymphoma formation has been observed around the sheath of some implants, it has been revealed that this situation is related to the implant structure and these implants have been removed from use.

9. What brand of implant do you use?

I am currently using Mentor and Motiva brand implants. After explaining the advantages and disadvantages of both to my patients, we make a joint decision with my patient. Both brands provide lifetime warranty.
